Analysis of Bottlenecks in a Human Hazard Reporting System

Abstract
Hazard reporting form the basis of the safety culture of any organization. Different industries
have different types of techniques which they have built for themselves for the reporting of
hazards. The major theme on which these systems work is behavior based safety. These
systems involve the active participation of all the employees who are trained to report any
kind of hazards that they see in their surrounding workplace so that they can be resolves at
the earliest. But there are some bottlenecks which are present whenever a system is being
designed which affect the effective and efficient performing of this process. Here in this paper,
authors have tried to categorize all those possible reasons which can affect a hazard
reporting system (by human) with the help of a survey conducted in an Indian based Oil and
Gas industry and respective measures to overcome such bottlenecks have also been discussed.
